Application Forms and Materials

(Please note that although you are able to print, complete, and mail in these forms, electronic submission of these forms is not currently available)

An application is considered complete when the following criteria are met:

  • The applicant has attended an Information Session and has met with a Nursing Advisor
  • Application materials are submitted on or prior to the deadline date
  • The following materials are present in the applicant's file
    • Personal Information Form (Application Form – Part 1)
    • Intent to Enter Nursing Form (Application Form – Part 2) – submitted no earlier than 2 quarters prior to the date of anticipated entry
    • Password for applicant's background check – completed within 30 days prior to the deadline date
    • Passing scores for the Pre-Nursing Arithmetic Test or acceptable grade for Math 17 (2.0 or greater)
    • Official high school transcript showing date of graduation and grades for high school requirements of algebra and chemistry (or college equivalents)
    • Official college transcripts showing grades for required prerequisite courses
      • Minimum cumulative GPA 3.0 (in required college courses)
      • Average Science GPA (in required science courses) of:
        • 3.0, if no science course was repeated
        • 3.15, if transcripts include any one science repeat
        • 3.30, if transcripts include any two or more science repeats
          (Note: Grades of N, U, I, V, W, E, or F are all considered a repeat, regardless of the student's individual circumstance)
    • Demonstration of the Essential Qualifications via a ‘Satisfactory' rating on the Interview and on a review of the student's file

The selection process is precise and is carried out by the Nursing Faculty as required by Washington State WACs. The process includes an academic file review and an interview focused exclusively on the Essential Qualifications.
